Does it give +5k faction cap regardless of Commander rank ?
I earned Codex Initiate and didn't get +5k

Yes. Codex gives you a +5k Balthazar cap regardless if you have Commander tiers. The only thing that they don't "mess well" together with is with you rHall of Monuments (which is still not even really worth mentioning when asked about the +5k cap).
